What is Hospice Care?

Hospice focuses on caring for people nearing the end of their life. Hospice Care is available to anyone who has a terminal or life-limiting illness, and has been given a prognosis of six months or less.

Typically, a family member serves as the primary caregiver and when appropriate, helps to make decisions for the terminally ill individual. Members of the hospice team make regular visits to assess the patient and provide additional care or other services. The hospice team develops a care plan that meets each patient’s individual needs for pain management and symptom control.

The team may consist of: a patient’s personal Physician, a hospice Physician, Nurses, Hospice Aides, Social Workers, Spiritual Counselors and hospice-trained Volunteers; Speech, Physical, Occupational, and Massage Therapy available as needed.
